He was an F-302 pilot, the leader of blue squadron during the dogfight against Anubis's forces over Antarctica. (Avalon part 1)
During the fight his 302 was hit. He still managed to save SG-1's collective asses by taking out an al-kesh headed straight for them before he crashed into the ice. (Avalon part 1)
He transferred to the SGC proper to become part of SG-1, hoping to serve under Sam's command. He was startled, to say the least, to find out that he had command of the unit -- and that all of the original members had moved on. He put most of his effort into getting Daniel, Teal'c, and Sam back, although he also went through at least some of the stack of personnel reports and interviewed some applicants for the team. He wasn't especially impressed with any of them. (Avalon part 1)
Still with no actual team of his own, Mitchell was assigned to take SG-12 and visit P3X-421 to check out a 'strange new missionary' who'd arrived and begun preaching a new religion, complete with miracles to prove his claim. He was the first Prior to visit our galaxy. (Origin)

I only count it if someone checks and says 'he's dead', or if someone gets zatted twice, or it's otherwise perfectly clear that there's no way this person is getting back up without being brought back from the dead.
